Pregnancy outside the uterus

Sometimes pregnancy can occur outside the uterus.
It is called as ectopic pregnancy.
In this case fertilized ovum can get implanted in extrauterine structures like fallopian tube or in ovary etc.

Early diagnosis of ectopic pregnancy is very important.
Because delay in diagnosis can lead to complications like rupture, heavy intraabdominal blood loss, shock etc.
Examination and ultrasonography at first visit of pregnancy means at around 5 to 6 weeks helps in ruling out the possibility of ectopic pregnancy.

Infertility in females

Having children in the home is considered as a boon.
It is true. But criticizing the couples who are not having children is not correct.
Usually women will face more problems related to it.

Before tagging a woman to be infertile, the points to be remembered are:
- Infertility is defined as not being able to get pregnant despite having frequent, unprotected sex for at least a year for most people and six months in certain circumstances.
-Generally female is 33% responsible for infertility, male for 33% and both for remaining 34%. So it is not correct to blame the female only.

Emergency contraceptive pill

Emergency contraceptive pill should be reserved for emergency purpose only.
Like in cases of:
- Rupture of condom
-Intercourse during the time of two missed contraceptive pills
-Unplanned intercourse at the time of ovulation
-Sexual assault etc.

Emergency pills should not be used as regular contraceptive method.
There should be at least two months gap between two emergency contraceptive pills.
Repeated intake of emergency contraceptive pills can lead to menstrual irregularities and can affect fertility also.
Side effects of emergency contraceptive pill

Choosing a suitable contraceptive method

There are many contraceptive methods available.
Women frequently get confusion regarding the method to be choosen.

It depends on factors like:
-Frequency of intercourse
-Whether the woman needs protection from sexually transmitted diseases or not
-How long she wants to postpone the pregnancy
-Family completed or not
-Presence of pelvic infection
-Any associated medical problems like liver disease, coagulation abnormalities, heart problems etc
-Breastfeeding or not

According to above factors:
-For women having infrequent intercourse, barrier contraceptive methods like condoms are useful.
-For women who want to avoid risk of sexually transmitted diseases also barrier contraceptive methods are advised.
-For young women who are having frequent intercourse, without any medical risk factors, who want to postpone the pregnancy for one to two years, oral contraceptive pills, monthly hormonal injections etc are advisable.
-For women who wants gap between two pregnancies, intrauterine contraceptive devices are useful.
IUCDs can give protection upto ten years depending on the type used.
To use IUCDs, pelvic infection should be ruled out.
-For women who have completed their family, permanent family planning methods like tubectomy are advised.
-For women having medical complications like coagulation abnormalities etc, barrier contraceptive methods are advised.
-For breast feeding women, progesterone containing low dose pills or progesterone monthly or once in three months injections are useful.

Hysterectomy at younger age

Hysterectomy is the surgery done to remove the uterus.
It should be done for conditions that cannot be managed medically and in emergency life saving situations.
But nowadays incidence of hysterectomies is being increased.
And most of them are being performed for benign, medically manageable conditions.
Some women think that after completion of the family, there is no role for uterus and if they underwent hysterectomy, they can get rid of problems of menstruation.

But this is not true.
Women undergoing hysterectomies under the age of 35 years can face many problems.
Ovaries are the organs which are responsible for the production of sex hormones estrogen and progesterone.
These are hormones will act on uterus and maintain regular menstrual cycle.
These hormones also help in cardiovascular function, in strengthening the bones etc.
Though ovaries are preserved in most of the hysterectomies, as the end organ is not there, the functioning of ovaries will also get affected.
That can lead to complications like:
-Decreased bone mineral density leading to low backache, body pains, join pains, generalized weakness etc.
-Hormonal abnormalities leading to mood changes, night sweats, hotflushes etc.
-Cardiovascular abnormalities ranging from palpitations to stroke
-Pelvic muscular weakness leading to vault prolapse, urinary incontinence etc.
-Psychological effects like decreased sexual desire etc.

These symptoms are more common and occur early in women who have undergone oopherectomy along with hysterectomy.
So, most of these problems can be prevented by choosing hysterectomies only for selectively indicated reasons.

Unwanted pregnancy

Sometimes accidentally because of failure of contraception or some other cause woman may become pregnant.
And she may want to avoid this unwanted pregnancy.

Here the facts should be remembered that:
-Usually woman can conceive if intercourse occurred around the period of ovulation.
-Emergency pill should be taken within 72 hours of the intercourse to avoid pregnancy.
-Sometimes insertion of intrauterine contraceptive devices can avoid pregnancy if inserted within 5 days of intercourse.
But this is preferable when the woman want to continue contraceptive effect of IUCD.
-If the woman crosses 72 hours after intercourse, usually prevention of pregnancy with tablets is not possible.
-In that case she has to wait, till her expected date of menstrual period.
-If she crosses the date menstrual of period, she has to check the possibility  pregnancy with urine pregnancy test.
Usually urine pregnancy test gives accurate result after 4 to 5 days of missed period.
-Medical termination of pregnancy is possible upto 8 weeks of gestation.
-To take tablets to induce the abortion, woman should:
•Consult gynecologist, get examined and undergo ultrasound to confirm the intrauterine pregnancy, to rule out ectopic pregnancy and to estimate the gestational age.
•Taking tablets without ruling out ectopic pregnancy can lead to complications like rupture of the organ containing ectopic pregnancy etc.
-If woman crosses 8 weeks of gestation, sometimes need of minor surgical procedure may arise.
-Usually termination of pregnancy is not advisable after 20 weeks unless there is specific reason.

Irregular periods

Many women complain that they are having irregular periods.

Before judging periods to be irregular, the criteria of normal periods should be known.
They are:
-Generally cycle length between 21 days to 35 days.
If a woman is having regular menstrual cycles with 45 days duration, that is normal for her.
-Menstrual flow for 3 to 7 days.
Usually a woman can loose around 80ml of blood in one menstrual cycle.

If any deviations from above criteria occur those can be considered as irregular periods.
The usual causes of irregularity are:
-Hormonal imbalance due to alterations in diet, changes in exercise schedule, changes in weight, stress and stain etc.
-Poly cystic ovary disease
-Thyroid hormonal abnormalities
-Abnormalities in prolactin levels
-Recent illness or usage of medicines
-General conditions like anemia, diabetes etc.

Self breast examination

Breast carcinoma is one of the common carcinomas among women.
By following methods like self breast examination, periodic mammography etc, most of the breast cancers can be detected early.
Self breast examination is a easy and important measure to detect breast cancer early.
Self breast examination should be done monthly at same period of menstrual cycle preferably after completion of menstrual cycle.
This is to avoid mistakes due to hormonal influenced breast changes.
Woman should do self breast examination by standing in front of mirror. She should use the pads of her fingers to palpate the breast tissue.

First she should observe her breast in positions:
-Arms hanging beside the body
-Arms behind the head
-Arms keeping on waist and bending forward.
-Palpation of breast in circular movements or in longitudinal movements or in wedge pattern.
Breast tissue should be palate upto axillary tail.
-Palpation of nipple
-Palpation of breast and nipple in lying down position.

If any abnormality is detected, she should consult the doctor.

Leuchorrhea

Leuchorrhea or white discharge is a common complaint among women.

First it should be remembered that white discharge may not be pathological always.
Physiological Leuchorrhea can be seen in conditions like:
-Just before periods
-Around the time of ovulation
-During pregnancy
-During intercourse or with it's thought etc.
Usually physiological leuchorrhea will not have any smell or color.
It will be like watery secretion without staining inner wear.
Sedentary lifestyle, constipation etc can also lead to leuchorrhea.

If white discharge presents with symptoms like:
-Foul smell
-With altered color like curdy white, yellow, green etc
-Itching presents
-Any associated genital lesions are seen
That can be considered as pathological and needs detailed evaluation.

Safe period

Knowing the safe period for intercourse in a menstrual cycle can avoid most of the unwanted pregnancies.
This is more applicable for women with regular periods.
Usually ovulation will occur in the middle of the month.
And generally postovulatory period will remain constant, that is around 14 days.
So, in case of regular 28 days menstrual cycle, ovulation is expected on and around 14th day.
By maintaing abstinence between 10th day of the cycle to 18th day usually pregnancy can be avoided.
In case of regular 30 days menstrual cycle, ovulation is expected on and around 16th day.
By maintaing abstinence between 12th day of the cycle to 20th day usually pregnancy can be avoided.
Sometimes length of menstrual cycle can fluctuate between 26 to 32 days.
In that case abstinence between 8th to 22nd day can avoid pregnancy effectively.
On counter available ovulation kits will also help in detecting fertile period.
Cervical mucus method and basal body temperature methods are other natural methods of contraception.

Hair loss after delivery

Usually women think that they are loosing more hair after delivery.
But this is not true always.

Actually during pregnancy under hormonal influence the hair will remain in growing phase only and the hair loss will be less.
After delivery as hormonal levels come back to pre pregnancy range, the hair will come out of growing phase and hair fall will start.
By 20 weeks after delivery normal hair fall and hair growth cycle will be restored.
Generally loss of up to 100 hair strands can be considered as normal.
Nutritious diet containing essential vitamins and minerals is important for the growth of hair.

Calcium to baby from mother

Do you know how much calcium the fetus take from mother?
It is around 3000mgs.
That is from the bones of the mother.
So to fulfill these needs, the mother should take more calcium.
Though dietary products can supply some amount of calcium, supplementation is needed to fulfill the needs of mother and baby.

Calcium supplements are usually advised after 12 to 14 weeks of pregnancy as calcium needs of the baby increases after this gestational age.
During first half of the pregnancy the woman should take around 500mgs of calcium per day.
In the second half of the pregnancy and during lactation around 1000 to 1200mg per day supplementation is needed.
Calcium supplements should not be taken along with iron or zinc or magnesium, as these can affect the absorption of calcium.
Some women may think that lactation may decrease their calcium levels.
During lactating period, the lactating women may have low calcium levels compared with nonlactating women.
But after lactation usually the women who have breastfed will have more bone mineral density compared with nonlactating women.
So breast feeding is advised always.

Is there any gender difference to get cold.
Actually no, but as women will spend more time with children, cold is common in women compared with men.
Because school going children are more prone for cold as it is a contagious disease.
